An exciting opportunity has arisen to join Maidstone Hospital team with the responsibility of managing clients located in the Romney Unit.

This involves assessing clients during their stay on Romney. When assessing clients there should be an emphasis on promoting independence and where possible clients’ needs should be met through our enablement services. The key to a successful outcome for a client is a well coordinated hospital discharge plan that incorporates the decisions made by the client and the multidisciplinary team. The case manager’s role is to coordinate this plan and ensure that the client choices are recognised and the discharge is safe.

You will have the ability to prioritise your work and manage a range of complex cases ensuring that clients are provided with an opportunity to meet their potential. You should be able to provide guidance and support to clients and their relatives to enable them to make decisions about services that they will require to meet their assessed needs. You must have a practical knowledge of the Mental Capacity Act and Safeguarding Vulnerable Adult procedures.

We will provide an induction programme, regular supervision and training.

This is a temporary position until 31/03/14.
