Case Manager

We have a vacancy for a Case Manager to work in the Swale Coordination Team. We are looking for a confident and competent team member, who can work autonomously as well as part of the team supporting service users and their carers in planning their future care needs. You must be either a qualified registered Social Worker, Occupational Therapist or Nurse.

You will bring with you excellent assessment skills and be able to use a person centred approach in supporting and coordinating the Care needs of our clients. Current knowledge of Care Management and Safeguarding vulnerable adults will be essential in your role. You will work closely with health colleagues and other providers to maximise wellbeing and choice for service users.

We provide an induction programme and ongoing training and development is available and actively encouraged.

We are committed to service user and carer involvement and seek to involve service users and carers in our recruitment processes.

This post is for a 3 year period only and subject to an Enhanced Disclosure Application to the Disclosure and Barring Service.
