We are looking for qualified clinical professionals (e.g. nurses, physios, OTs, health visitors, etc) to deliver this innovative model of care in the Ashford, Canterbury and Margate area to create measurable improvements in population health. As an individual, you are results-orientated and enjoy engaging with, motivating and enabling patients to prevent avoidable future health care needs. You are a good listener and are able to quickly understand a patient's situation in a holistic manner. Company Background Health Navigator is a provider of Proactive Health Coaching (PHC) - a proactive, targeted mostly telephone-based, health service. It is designed to measurably improve quality of life and ability to self-manage for patients resulting in a reduction in unplanned acute care and increased quality of life for patients with complex health and social care needs. A health coach, a specially trained nurse or allied health professional (AHP), helps patients to increase their patients' participation and understanding of their health conditions and to ensure a well-functioning health and social care pathway. The intervention begins at an early stage of the disease progression and is an individualised time-limited programme for each patient. PHC primarily works alongside primary care and pulls together resources, e.g. specialists, social care and voluntary sector, to cover all of the patient's needs, whilst promoting self-care and patient education. Our model originates from scientific research of case and disease management and is supported by several randomised trials. Health Navigator is an organisation founded by physicians and researchers from the Karolinska Institute in Stockholm, Sweden. Health Navigator specialise in the provision of innovative health care services to achieve measurably improved outcomes for vulnerable patient groups, where one such service is Proactive Health Coaching (PHC). PHC has been provided to over 30,000 patients across a population of six million, 17 hospitals and 450 primary care centres in Scandinavia since 2010 and in York, UK since 2015. The intervention has delivered 20-40% reductions in unplanned hospital activity within the target patient groups. Over 90% of the patients say that their quality of life has increased, and 85% are either satisfied or very satisfied. The results have been published in two articles in the European Journal of Emergency Medicine. Health Navigator employs more than hundred people and has offices located in Stockholm, Copenhagen and London. The Role Proactive Health Coaching is a mainly telephone-based health service that helps patients understand their health conditions, plan their care, and find their way around the healthcare system. The post-holder will: Meet patients and jointly develop an action plan for how their health and quality of life can be improved and future care needs prevented. This is mainly delivered by telephone over a number of months following an initial face-to-face meeting. Coach and motivate patients to follow their individualised plan and care pathway, supporting them to make proactive choices. Engage and effectively communicate with other health, social care and voluntary sector providers to holistically support the patients' health and social care well-being. Share and learn experiences as part of a team, focusing on specifically challenging patient cases. Participate in regular meetings focusing on continuously improving quality and implementing improvements.
